Mechanisms for the development of heart failure and improvement of cardiac function by angiotensin-converting enzyme inhibitors

Angiotensin-converting enzyme (ACE) inhibitors, which prevent the conversion of angiotensin I to II, are well-known for treatments cardiovascular diseases, such as heart failure, hypertension and acute coronary syndrome. Several these inhibitors including captopril, enalapril, ramipril, zofenopril imidapril attenuate vasoconstriction, cardiac hypertrophy adverse remodeling, improve clinical outcomes in patients with dysfunction decrease mortality. Extensive experimental research over past 35 years has revealed that beneficial effects ACE failure associated full or partial prevention remodeling. Since function is mainly determined by coordinated activities different subcellular organelles, sarcolemma, sarcoplasmic reticulum, mitochondria myofibrils, regulating intracellular concentration Ca2+ myocardial metabolism, there ample evidence suggest remodelling failing consequence defects. In fact, improvement been demonstrated be related attenuation abnormalities organelles Ca2+-handling, metabolic alterations, signal transduction defects gene expression changes cardiomyocytes. Various have also shown delay progression reducing formation development oxidative stress, level inflammatory cytokines occurrence These observations support view multiple mechanisms reduction inflammation Ca2+-handling
